Registration for the 2010 Season

U9 U-11 & U-15 ARE STILL ACCEPTING REGISTRATIONS: 

Please call 617-312-0831 to register

Download and send the forms(2) with registration fee back to our PO Box.  

If you know of other boys and girls* from the Parkway area that wish to play please inform them how can get registration forms and mention that they do it ASAP.

Registration Rates for 2010

Boys U-9 to U-15  $135 per player

(send all checks with forms to Parkway Lax- NOT USLAX!!)

            * Girls are allowed to play Boys U-9 & U-11

Instructions for Filling out Registration Form – US Lacrosse –

  1. Please print legibly – Blue or Black Ink only

    2.   US Lacrosse form – please fill out as completely as possible

           Top Section – asks for Player’s name, address, date of birth, etc             

           Step 1 check Youth

           Step 2 check Player

           Step 3  IGNORE – check should be made payable for the appropriate fee

 Boys  $135   (Family Maximum $300)  Do not mail to USLAX

      3.   Please fill out all Insurance information and make sure parent or guardian signs form

Coaches – Please mark form as a coach and send back to Parkway Lacrosse – please do not register directly with US Lacrosse

Please fill out both forms (US Lax &Parkway Registration form) for additional information, extra email addresses and contact information. Please include tee shirt & short sizes

U-11: Grade 3&4

U-9: Grade 1&2**

Girls are eligible to play with the boys U-9 & U-11 teams

**U-9- some "K2" grade are eligible-- Contact Board member for details

Fall Instructional Lacrosse program at the West Roxbury YMCA
Lacrosse (Ages 5–7) New program

Lacrosse is the fastest growing sport in the country!
In this co-ed instructional class your child will learn
the fundamentals of the game including cradling,
catching and throwing.

Ages 5-7
starting the week of November 1st
